FISP Close-Out and Amended Reports in NYC

The repairs aren’t finished until DOB’s record says so. We pull together the photos, permits and sign-offs your QEWI needs to file the amended report, so the Unsafe status clears, the shed can come down and the penalties stop.

What Does FISP Close-Out Involve?

FISP close-out is the paperwork side of a facade repair. For Unsafe conditions, DOB’s facade rule requires your QEWI to inspect the work within two weeks of completion, then file an amended report showing the building’s new status. Until DOB accepts that report, the building stays Unsafe, the protection stays up and the monthly penalties keep running.

Close-out also means signing off every permit the work used, including the shed permit, and posting the new conditions certificate. That certificate goes up in the lobby or vestibule, in a frame with a clear cover, within 30 days of issuance. SWARMP items don’t need an amended report, but an optional subsequent report can update your status mid-cycle.

FISP Close-Out Records to Pull Together

  • The filed report and DOB's notice of the Unsafe rating
  • Dated before-and-after photos of every corrected location
  • Work permit numbers, job numbers and their sign-off status
  • The sidewalk shed or netting permit and its expiration date
  • Any extension approvals granted during the repairs

How We Support Your FISP Close-Out

  • Completion walk-through

We walk every repaired location with your QEWI to confirm nothing on the list was missed.

  • Photo log by location

Dated photos matched to the location references in your report.

  • Permit sign-offs

We close out the work permits our scope used and request final inspections where needed.

  • Protection removal plan

Shed and netting removal is lined up for the day DOB allows it.

  • Fixes after a rejection

If DOB rejects the amended report, we fix any open item quickly so the revised report goes in inside 45 days.

  • Hand-off file

Everything organized for the next cycle's QEWI.

Amended Report Mistakes That Keep a Building Unsafe

Waiting to Book the QEWI Inspection

The inspection is due within two weeks of finishing the repairs. Book it before the crew demobilizes.

Leaving Work and Shed Permits Open

Penalties run until the shed and related permits are signed off, not just until the work is done.

Repair Photos Without Locations

Photos that can't be matched to the report's locations slow the review. Label every one.

Taking the Sidewalk Shed Down Early

The shed stays until DOB accepts the amended report, unless DOB approves early removal.

What Goes Into a FISP Close-Out Package

A complete package is what turns finished repairs into a cleared record. Here are the five parts DOB and your next QEWI will look for.

  1. QEWI Inspection and Amended Report

The QEWI inspects within two weeks of completion and files the detailed amended report with DOB, along with the $425 filing fee.

  2. Photo Record of Every Correction

Clear, dated color photos for each location that was listed as Unsafe or SWARMP.

  3. Permit and Job Numbers With Sign-Offs

Every work permit closed out, including the sidewalk shed permit.

  4. Shed Removal Plan or Early Removal Request

Removal is scheduled for after acceptance, or your QEWI submits a signed and sealed statement asking DOB to allow it sooner.

  5. Updated Conditions Certificate

The new certificate is posted in the lobby or vestibule, framed with a clear cover, within 30 days of issuance.

What Affects FISP Close-Out Time and Cost

Close-out is mostly paperwork, but delays cost real money. These five things decide how fast and how cheaply it goes.

  • Filing fee

$425 for each amended or subsequent filing, per DOB's fee schedule.

  • Rejections

A rejected amended report needs a revised filing within 45 days, and a new fee applies after two rejections.

  • Open permits

Permits left open from earlier jobs can hold up the final sign-offs.

  • Shed time

Every month the shed stands after the work is done adds more rental cost.

  • Record gaps

Missing photos or job numbers mean extra site visits to rebuild the file.

FISP Close-Out and Amended Report FAQs

Who files the amended FISP report?

Your QEWI does. The QEWI inspects the finished work, prepares the amended report and files it with DOB. The owner pays the filing fee and keeps the records.

How much does it cost to file an amended FISP report?

DOB's filing fee is $425 for each amended or subsequent filing. The QEWI's own fee and any remaining repair work are separate.

What happens if DOB rejects the amended report?

The QEWI has 45 days to file a revised report. After two rejections, a new amended filing fee applies, so it pays to fix every open item before refiling.

Do Unsafe penalties stop when the repairs are finished?

No. Under DOB's rule, the penalty runs until DOB receives an acceptable amended report and the shed and related permits are signed off.

Where does the FISP conditions certificate have to be posted?

In the lobby or vestibule, in a frame with a clear cover, within 30 days of issuance. It shows the most recent condition of the building's facade.

What is a subsequent FISP report used for?

A subsequent report is filed within the same five-year cycle to change the building's filing status or the recommended repair timeframe. A new timeframe needs supporting documents from the QEWI.
